To schedule a new patient visit, you can either call our office at 864.233.5722 and select option 2 for Referrals or you can schedule online. For all urgent referrals, please call the office for the quickest response. The online scheduling tool should only be used for non-urgent appointments.
If you schedule online please fax a completed Patient Referral Form and last exam note to our office at 864-233-6027. If you don’t see the patient’s insurance listed on the online scheduling portal please call our office to confirm our participation with that plan.
If you are scheduling from a provider’s office, please enter the provider’s name and office name in the appointment notes.
